I consent to the processing of my personal data for the needs necessary to organize the conference. Information on the processing of personal data at the University of Bialystok
According to Art. 6 sec. 1 lit. a of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (GDPR) I consent to the processing my personal data by the University of Bialystok (co-organizers and co-administrators), in order to organize and participate in the conference entitled What the crisis has taught us ? I am aware that the consent may be withdrawn, however, the withdrawal of the consent does not affect the lawfulness of the processing which was made on the basis of the consent before its withdrawal. I provide my data voluntarily and ensure that it is correct.
According to Art. 6 sec. 1 lit. a GDPR and art. 81 of the Act on Copyright and Related Rights, I declare that I consent to the free processing and dissemination of my image and voice in connection with my participation in the conference entitled What has the crisis taught us? organized by the University of Bialystok (co-organizers and co-administrators). The consent authorizes the recording and dissemination of my image and voice for information and educational purposes, by making it available without time and territory limits, in the mass media, including social media. Thus, I agree to make changes and modifications to my image necessary for its dissemination, consisting in its graphic processing (retouching, changing the cropping and other similar modifications) and incorporating the image into other works, provided that they cannot distort my image. Such changes and modifications do not require a separate consent.
